备胎 literally means 'spare tire' — just like a spare tire in the trunk, a 备胎 is someone kept around 'just in case' the main relationship doesn't work out.
usage
当备胎 or 做备胎 = to be someone's backup. Can also be used as a verb: 被备胎了 = to be relegated to backup status. Very common in dating discussions.
