土包子 is colloquial and mildly derogatory. It can be used self-deprecatingly (as in the second example) or to tease someone. It implies someone is naive about urban customs or fashions, not necessarily from the countryside literally. 土 means 'earthy/rustic' as opposed to 洋气 (fashionable/cosmopolitan).
