马上 literally 'on horseback' — to do something
马上 meant to do it immediately, the way a mounted messenger arrived.
马上来 is the modern service-industry reply of choice: restaurants, couriers, plumbers. Often understood to mean 'in a few minutes,' not literally instantly.
