Older / more literary than 疯子 (fēngzi, the everyday word for 'madman'). Most common today inside compounds: 疯人院 'asylum', 疯人节 (an old name for April Fool's Day in Hong Kong/Taiwan).
mistakes
Reserve for either fictional/literary contexts or set compounds. Calling someone 疯人 to their face would sound stilted; 你疯了吗 'are you crazy' is the natural insult.
