后魏 (Later Wei) and 北魏 (Northern Wei) refer to the same dynasty (386-534 CE), founded by the Tuoba 拓跋 clan of the Xianbei 鲜卑 people. The 后 prefix distinguishes it from the earlier Three Kingdoms 魏 (220-266 CE) of Cao Cao's lineage.
