From Han-dynasty musician Li Yannian's song quoted in 《汉书·外戚传》: 北方有佳人，绝世而独立 — 'In the north there is a beauty, peerless and standing alone.' He was singing about his own sister, who became Emperor Wu of Han's beloved consort Lady Li.
usage
Always describes a woman, almost always in literary or admiring tone. Modern equivalent in casual speech is just 大美女 (dà měinǚ).
