From a Han-dynasty song by the musician Li Yannian, quoted in 《汉书·外戚传》: 北方有佳人，绝世而独立 - 'In the north there is a beauty, peerless and standing alone.' He was singing about his own sister, who became Emperor Wu of Han's beloved consort Lady Li.
usage
Always describes a woman, almost always in a literary or admiring tone. The modern equivalent in casual speech is just 大美女.
