Literary/formal. Most commonly heard in set phrases 别来无恙 (a classical greeting) and 安然无恙 (safe and sound). Rarely used alone in casual speech.

history

恙 originally referred to a type of illness-causing insect. 无恙 literally meant 'without the illness bug,' hence 'in good health.'
